Beaverton Clinic Corner

VG Beaverton celebrated its seventh year of operation on March 1.  “We’ve grown tremendously in the past several years,” said clinic manager Tran Miers, “and we’re continuing to grow and improve our services.”  VG Beaverton currently serves more than 7,500 patients and scheduled more than 24,000 office visits last year.

In the fall, VG Beaverton joined with CareOregon and Oregon Senior and Disabilities Services in a long term care pilot project.  The project identified patients who may not be receiving the medical care they need and referred them to case managers for coordinated care.  “The grant we received for this project allows us to send a nurse practitioner to the patient’s home to be our ‘eyes and ears’ and report back to the patient’s primary care provider,” said Miers.  “We believe this coordinated care will help reduce the need for more expensive acute care.”

VG Beaverton is also participating in the Care Transitions Innovation pilot developed by OHSU to track patients after they are discharged from the hospital.  “Research shows that there is a high occurrence of patients without insurance being readmitted to acute care within the first week after leaving the hospital,” said Miers. “We are assigning case managers to help patients connect with a Virginia Garcia primary care provider and to follow up with their progress.”  The Care Transitions Innovation project is a one year pilot.
